uses a team based approach to care that uses the most current evidence to support decision making
about individual circumstances. Springs of Joy provides person centred care with the involvement of
the service user and diligently managing the whole process to ensure progress is being made every
step of the way.
We aware that our outcome- based approach is important for a number of reasons but mainly
because our clients are happier and their wellbeing is being placed at the centre stage. We follow
the guidelines from the Department of Health’s White Paper ‘Our Health, Our Care, Our Say.

We support our clients, families and friends to use either of the above processes when setting and measuring personal outcomes for their personalised support plans.
We maintain a high standard recording of our information where we documents our client’s
outcome at different stages, meaning that everything is there in writing and information can be fed
into care plans for individuals so its always up to date.
Through documenting and monitoring outcomes, we are able to identify changes in care. This also
gives us an opportunity to identify and monitor any gap in training skills for our care staff.
We use any feedback- both positive and negative to communicate with the carers and to help them
to improve and develop their skillset which also form part of their training plan.
We aim to working as one team where our service users and their families are given opportunities to
make more informed care decisions together. Plans for each service user are always developed
based on their specific needs with their input setting goals and reviewing treatment outcomes.
